Sexual Health – Emergency Hormonal Contraception (EHC), Chlamydia screening and C Card.

Smoking Cessation

Domiciliary Stop Smoking Service

HLP level 2 for HLPs in Brighton and hove

Substance Misuse and Needle Exchange services – Pavilions are delivering the Substance Misuse Service for Brighton & Hove from 1st April 2015.

Brighton & Hove City Council (BHCC) have issued new contract documents and updated service specifications for locally commissioned services provided in Brighton and Hove.

The deadline for return of contracts to BHCC is 30th April 2016.

This applies to: Smoking Cessation, EHC and Sexual Health Services, Domiciliary Stop Smoking Service. Also the HLP Level 2 Service Specification which only applies to those pharmacies who are ‘Healthy Living Pharmacies’ in Brighton and Hove.

Cluster Map – for the six GP Clusters, (Proactive care Brighton and Hove practices), click to locate your nearest GP cluster.

LCS outcomes framework July 2015 FINAL -The LCS contract is offered to general practices working at Cluster level. Baseline assessment for each practice will be provided for the outcomes in the framework. The contracting approach will be one of a strategic partnership with the objectives of commissioner and providers fully aligned. The commissioner will set out the outcomes and invite clusters to submit costed action plans with targets using baseline assessment.
